What are the side effects of Metformin HCL Lotion 20%?

Metformin HCL Lotion 20% is generally well-tolerated because it bypasses the digestive tract — meaning the gastrointestinal side effects common with oral Metformin don't apply.

Most common side effects:

  • Skin irritation at the application site mild redness, slight dryness, or itching. Often resolves as your skin adjusts.

Less common side effects:

  • Application site reactions sensitivity, peeling, or mild burning sensation, particularly with frequent or thick application.
  • Rarely, systemic effects — because some topical metformin is absorbed into the bloodstream, very rare effects similar to oral metformin are possible at higher doses or with extensive skin coverage. These are uncommon at typical prescribed doses for PCOS-related skin conditions.

A note on what Metformin HCL Lotion 20% doesn't typically cause:

  • The GI side effects of oral Metformin (nausea, diarrhea, stomach upset) are not typically a problem with topical application.
  • The risk of lactic acidosis associated with oral metformin is extremely rare with topical use given the limited systemic absorption.
  • Vitamin B12 absorption issues significantly less relevant for topical use than oral use, but worth mentioning to your clinician if you're using both.

What to do:

  • Most skin reactions are mild and improve as your skin adjusts. Continue use unless the reaction is significant or worsening.
  • Apply a thin layer rather than a thick one more isn't more effective and can increase irritation.
  • Talk to your clinician about other actives you're using in the same area. Layering with retinoids, acids, or other prescription topicals can compound irritation.
  • Message your clinician if irritation is persistent or significant.
  • Seek immediate care for signs of allergic reaction (facial swelling, difficulty breathing, severe rash).
