What is Metformin ER?

Metformin ER is the extended-release version of Metformin — a long-established prescription medication that's the first-line treatment for type 2 diabetes and the most commonly prescribed insulin sensitizer for PCOS.

A bit of context on Metformin itself. It's one of the most-studied medications in modern medicine, in use for decades, with a well-established safety profile and a clear mechanism for what it does: it improves how your body responds to insulin, reduces the amount of glucose your liver produces, and helps your cells absorb glucose more efficiently. The result is lower chronic insulin levels and better blood sugar regulation — which, in PCOS, has downstream effects on the symptoms driven by insulin resistance and androgen excess.

"ER" — extended release — means the active ingredient is released into your body gradually over many hours, rather than all at once. This is different from regular (immediate-release) Metformin, which dissolves and absorbs over a shorter window. The extended-release formulation has two main practical benefits: fewer gastrointestinal side effects (which are the most common downside of Metformin generally), and a once-daily dosing schedule rather than two or three times a day.

For women with PCOS or PCOS-pattern hormonal imbalance symptoms, Metformin ER is often one of the first treatments your clinician will consider — particularly if insulin resistance appears to be part of the picture or if you're managing multiple connected symptoms (weight, cycle, acne, energy) where addressing the underlying mechanism has potential to shift several things at once.
